Orthodontics & Sleep Apnea in Children: What Parents Should Know

When it comes to your child’s growth and development, you may have heard conversations around orthodontics and sleep apnea. You might have even heard the emerging term “airway dentistry”. 

With so much information available, it can be difficult to know what’s evidence-based and what’s not. This article focuses on the role orthodontists actually play in assessing sleep-related concerns — and how their expertise factors into your child’s overall care.

How Orthodontics Relates to Sleep Apnea

Sleep-disordered breathing (SDB), which includes conditions like obstructive sleep apnea (OSA), exists on a spectrum.

Orthodontists are trained to assess facial growth, jaw development and dental structures. Our orthodontists, Dr. Martin Poon and Dr. Geoffrey Hall have the expertise to identify potential risk factors affecting breathing during routine examinations. 

However, it’s important to be clear:

  • orthodontists do not diagnose sleep apnea. A formal diagnosis must come from a qualified medical professional, typically involving a sleep study (polysomnography).

How Orthodontists Observe Signs of Sleep Apnea or Sleep-Disordered Breathing

During an orthodontic consultation, particularly in younger children, our orthodontists seek to understand your child’s overall development. 

This can include asking questions like:

  • What are your child’s sleep habits and breathing patterns?
  • Do they have a history of snoring or restless sleep?
  • Do you see any signs of fatigue or difficulty concentrating during the day?

If concerns arise, we may recommend a referral to a GP, paediatrician, or sleep specialist for further investigation. This ensures your child receives the right diagnosis and care pathway from the outset.

Myths About Orthodontics and Airway Health

There are common misconceptions that a child’s jaw, facial structure, anatomy and teeth contribute to breathing issues, such as: 

  • “A bigger airway on a scan means better breathing”
  • “A tongue-tie causes sleep apnea”
  • “Children outgrow mouth breathing and snoring”
  • “Expanding the jaw fixes breathing”
  • “Extracting teeth leads to sleep and breathing problems”

Or our favourite falsehood:

  • “Orthodontic treatment can fully cure airway issues”

These claims are not supported by strong scientific evidence. When recommending care, our orthodontists rely on research, not trends.

Airway Dentistry and Jaw Development – What the Evidence Actually Says

It’s easy to assume that jaw size or shape directly determines how well a child breathes during sleep. In reality, sleep-disordered breathing is complex and multifactorial.

Current research shows:

  • No clear causal relationship between craniofacial structure and sleep-disordered breathing
  • Airway function cannot be predicted simply by looking at jaw position or size

This is why our orthodontists take a cautious, evidence-based approach, avoiding oversimplified claims like: “a smaller jaw means airway problems.”

Understanding the Limits of Orthodontics in Sleep Apnea

There’s a growing amount of marketing suggesting orthodontic treatment can “fix” or “prevent” sleep apnea. However, current evidence does not support orthodontics as a standalone solution for sleep-disordered breathing.

Orthodontic treatment should never be used as a substitute for proper medical diagnosis or management. 

In fact:

  • There is no evidence that orthodontic treatment can prevent SDB from developing
  • Treatment decisions should only be made after a confirmed diagnosis, and often as part of a broader medical care plan
  • The focus should be on early assessment, appropriate referral, and evidence-based care

This is why ethical orthodontic care focuses on working alongside medical professionals, not replacing them.
